ircd: enforce registered channel ownership and expand services help

Require a new channel founder's current nickname to belong to an
authenticated NickServ identity, while retaining PASS authentication and existing registrations. Correct automatic creator ops when an empty
registered channel is recreated: founders and OP/SOP accounts keep ops,
VOICE accounts receive voice, and unrelated creators lose ops. Track the initial grant separately from bursts and later manual grants, and recheck policy and channel identity before applying queued corrections.

Add ordered, transactional services database migrations from deployed
schema 3 to schema 4. Preserve accounts, linked identities, registrations, access lists and exception grants, and roll back failed upgrades.

Provide detailed HELP COMMAND and subcommand help for NickServ, ChanServ
and MemoServ, including syntax, permissions, behavior and examples. Keep
help public and bound NOTICE replies to the IRC wire limit.

Cover policy and queued-action races, migration preservation and rollback,
help routing and framing, and native channel recreation through updated
servers and actual v1.3/v2.0 leaves. Channel-state and services regression tests pass.
